When was Care Resources founded?
Care Resources began with the vision of some key community leaders. They saw the need to bring PACE® to Kent County. The planning began in 2002 with five organizations partnering to fund the Care Resources project. Our community partners include:

- Holland Home
- Metro Health
- Pine Rest
- Reliance Community Care Partners
Care Resources successfully opened on October 1, 2006 and began providing PACE services. We owe our thanks to our community partners for starting PACE in Kent County.
Where is Care Resources located?
Care Resources is located at 4150 Kalamazoo Ave SE Grand Rapids, MI 49508. After 13 wonderful years at the Grace St. location, we moved to our new building on August 5, 2019.
What are your hours?
Our hours are Monday-Friday 8:00 am – 4:30 pm. After hours, and on weekends, we have a nurse, physician, and social worker on call.
How do I learn more?
The first step is to make a phone call to our Intake Team at 616-913-2006 or 800-610-6299. They will get to know you by asking some questions and determine if you are eligible. Care Resources is not an option for a crisis situation. It is best to start the process for enrolling into Care Resources sooner, rather than later, because the entire process may take up to 45–60 days. What does PACE stand for?
PACE® stands for “Program of All-inclusive Care for the Elderly”. PACE is a Medicare and Medicaid funded option that gives community-based care and services to people 55 years or older, helping to prevent nursing home placement.
How do I qualify for Care Resources PACE?
You may qualify for PACE® if you meet the following criteria:
- 55 years or older
- Resident of Kent County or these select zip codes in Allegan, Barry, Ionia and Ottawa Counties: 48809, 48815, 48846, 48849, 48865, 48881, 48897, 49058, 49302, 49315, 49316, 49323, 49325, 49328, 49331, 49333, 49344, 49348, 49418, 49428, 49435, 49534, 49544
- Able to live safely in the community when you join PACE
- Meet the State of Michigan nursing facility level of care

Is Care Resources affordable?
Yes. Care Resources is an established PACE® provider for people in West Michigan who have both Medicare and Medicaid coverage. Care Resources makes quality healthcare accessible and affordable. For those who qualify, there are no premiums, deductibles or co-pays for any of the services (unless a person goes into a nursing home for more than 30 days).
Who is the typical PACE participant?
According to the National PACE® Association’s website:
“The typical participant is an 80-year-old woman with eight medical conditions and limitations in three activities of daily living. Nearly half (49 percent) of PACE participants have been diagnosed with dementia. Despite a high level of care needs, more than 90 percent of PACE participants are able to continue to live in their community.”

Does Care Resources support family caregivers?
Yes. PACE® organizations support your family members and other caregivers with caregiving training, support groups, and respite care to help families keep loved ones in the community.
Does Care Resources provide prescription drug coverage?
PACE® organizations offer Medicare Part D prescription drug coverage. If you join a PACE program, you’ll get your Part D-covered drugs and all other necessary medication from the PACE program. For most who qualify, there are no premiums, deductibles, or co-pays for any service (unless a person goes into a nursing home for more than 30 days).
Note: If you are in a PACE program, you don’t need to join a separate Medicare drug plan. If you do, you will lose your PACE health and prescription drug benefits.
