Care Resources is a Program of All-Inclusive Care for the Elderly (PACE®), a national healthcare program funded by Medicare/Medicaid. We’re a community-based program for adults 55 years or older that promotes healthy, independent living and helps prevent nursing home placement. Our program is all-inclusive concierge care led by skilled primary care providers and an interdisciplinary team with 11 specialties represented. Care Resources embraces physical, mental, and social health, making healthcare accessible and affordable.
Position:
- Provide personal care such as bathing and dressing and other care services to participants including assistance with shopping, running errands, laundry, housekeeping, meal preparation, and recreational activities in homes.
- Work full time Monday-Friday with an every 4-6 week weekend rotation
Requirements:
- Valid drivers license, reliable vehicle, auto insurance, and a good driving record
- Ability to read, speak, write, and understand the English language
- Ability to stand for long periods of time and lift 60 pounds
- Pass a basic physical exam
- Pass a background check
Pay:
- Non-Certified starting at $19.51 per hour
- Certified starting at $20.51 per hour
- Bi-weekly pay checks / next day pay

Care Resources is adding another full-time Bus Driver to our team! This position works Monday – Friday days and is eligible for full-time benefits, paid time off, paid holidays and a generous retirement savings plan.
Our Bus Drivers are responsible for providing safe transportation services to the Care Resources participants according to assigned routes/appointment schedules and utilizing Care Resources vehicles. There may also be some delivery services as needed.

Physical Therapist
Location: Care Resources PACE – Grand Rapids, MI
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (no weekends!)

What You’ll Do
- Perform intake screenings and make appropriate recommendations for services.
- Function as a key member of the interdisciplinary team, collaborate on care planning and service allocation decisions.
- Complete participant evaluations as needed and develop plans of care.
- Provide treatment to participants and implement physical therapy procedures consistent in achieving identified goals.
What We’re Looking For
- Physical Therapist license in the state of Michigan
- Minimum of 1 year of experience working with a frail or elderly population
- Ability to multi-task, work independently as well as within a team setting
- Have a passion for those whom we serve through this amazing program

PCA | Dietary Aide | Universal Worker

About this Role: Are you passionate about helping others and making a difference in the lives of older adults? The Lowell Center is seeking a compassionate and dependable PCA | Dietary Aide | Universal Worker to join our team. This unique role combines food service responsibilities with personal care support, ensuring our participants receive nutritious meals and the respectful, hands-on care they deserve.
What You’ll Do
In The Day Center:
Assist participants with personal care (bathing, grooming, toileting, feeding)..
Support mobility and use of assistive devices..
Escort participants to activities and appointments..
Monitor and report changes in participant condition..
Help with recreation and physical activities..
Assist participants entering and exiting transportation vehicles
In the Kitchen
Prepare, assemble, and deliver meals based on dietary needs.
Operate and maintain kitchen equipment..
Clean and sanitize kitchen and dish room areas..
Stock pantries and manage food inventory..
Ensure food safety and complete required logs.
What We’re Looking For
High school diploma or equivalent.
Experience with elderly or frail populations.
Food service experience is a plus.
Ability to lift up to 60 lbs and stand for long periods.
Strong communication and teamwork skills.
Compassionate, reliable, and detail-oriented.
