Care Resources is a Program of All-Inclusive Care for the Elderly (PACE®), a national healthcare program funded by Medicare/Medicaid. We’re a community-based program for adults 55 years or older that promotes healthy, independent living and helps prevent nursing home placement. Our program is all-inclusive concierge care led by skilled primary care providers and an interdisciplinary team with 11 specialties represented. Care Resources embraces physical, mental, and social health, making healthcare accessible and affordable.
Position:
- Provide personal care such as bathing and dressing and other care services to participants including assistance with shopping, running errands, laundry, housekeeping, meal preparation, and recreational activities in homes.
- Work full time Monday-Friday with an every 4-6 week weekend rotation
Requirements:
- Valid drivers license, reliable vehicle, auto insurance, and a good driving record
- Ability to read, speak, write, and understand the English language
- Ability to stand for long periods of time and lift 60 pounds
- Pass a basic physical exam
- Pass a background check
Pay:
- Non-Certified starting at $19.51 per hour
- Certified starting at $20.51 per hour
- Bi-weekly pay checks / next day pay

Care Resources is adding another full-time Bus Driver to our team! This position works Monday – Friday days and is eligible for full-time benefits, paid time off, paid holidays and a generous retirement savings plan.
Our Bus Drivers are responsible for providing safe transportation services to the Care Resources participants according to assigned routes/appointment schedules and utilizing Care Resources vehicles. There may also be some delivery services as needed.

Care Resources (PACE) is seeking a full-time Recreational Therapy Assistant (RTA) who will be responsible for assisting the Recreational Therapist in planning, coordinating, and facilitating a variety of individual and group therapeutic and social activities for participants. Their main focus will be in relation to activity but may also be responsible for providing personal care and various other care services necessary for attending to the personal needs, support and health status of the participants of the PACE program at our location in Lowell, Michigan.
RTA/Activity Assistant Duties and Responsibilities:
- Assist in planning, coordinating, and facilitating a variety of individual and group therapeutic/social activities for Care Resources participants.
- Complete required documentation within the electronic medical record.
- Coordinate the efforts of volunteers assisting in the facilitation of activities.
- Transport participants within the center, to/from activities, and arrange activity rooms, as needed.
- Maintain general order in the activities area, including daily clean-up, and supply inventory.
Qualifications:
- The job requires knowledge normally acquired through completion of a high school diploma or equivalent.
- Previous RTA or Activity Assistant experience.
- Geriatric experience is preferred.
- A minimum of one-year experience working with a frail or elderly population.
- Must be able to multi-task and work independently as well as in a team setting.
- Experience in leading group activities.
- Must have current valid driver’s license and insurance, Michigan preferred.
- Must maintain current knowledge of and adhere to required regulatory compliance by governing entities, HIPAA and PACE specific regulations.

Full-time Schedule:
- Monday – Friday
- 8:00 am – 4:30 pm

As we continue to grow and serve the needs of our program participants, Care Resources is seeking to add a full-time Social Worker (MSW) to our growing team! This position works under the supervision of the Social Work Supervisor and is a critical member of the interdisciplinary team. This key team member provides direct social work case management services to participants in the Care Resources Program.
What will you do?
- Conduct initial psychosocial assessments on prospective program participants
- Consult with family or other support system members as needed
- Develop clear and timely written social service care plans and implements those plans; review and revise as appropriate
- Functions as a member of the interdisciplinary team
What is needed to be successful?
- A valid MSW license in good standing in the State of Michigan
- Minimum of 1 year of working with the frail or elderly population
- Valid driver’s license and reliable transportation are necessary to make home visits
