Care Resources is a doorway to living well at home. We’re a community-based program for adults 55 and older, helping you stay healthy, independent, and supported, while preventing nursing home placement.
A Team Approach to Care
Our Care Resources team partners with you and your family to create a personalized care plan that supports your health and independence. Care is provided through our Primary Care Clinic, Adult Day Center, and trusted community providers—guided by our team of physicians, nurses, therapists, and other specialists. Together, we open the doorway to your senior independence.
