Care Resources is a community-based program for people 55 years or older that promotes healthy, independent living and helps prevent nursing home placement. We have healthcare professionals who watch for changes in your condition, provide treatment, and encourage you every step of the way.
We are part of something bigger
Care Resources is a Program of All-inclusive Care for the Elderly (PACE). PACE is a federal- and state-funded program throughout the country. The PACE model is centered on the belief that it is better for you and your family to be served in your own community whenever possible.
PACE uses Medicare and Medicaid funds to cover all of your medically-necessary care and services. You can also pay for PACE privately if you don’t have Medicare or Medicaid.
PACE® eligibility
You may qualify for Care Resources PACE if you meet the following criteria:
- 55 years or older
- Able to live safely in the community when you join PACE
- Meet the State of Michigan nursing facility level of care
- Resident of Kent County or these select zip codes in Allegan, Barry, Ionia and Ottawa Counties: 48809, 48815, 48846, 48849, 48865, 48881, 48897, 49058, 49302, 49315, 49316, 49323, 49325, 49328, 49331, 49333, 49344, 49348, 49418, 49428, 49435, 49534, 49544
