Care Resources PACE is a doorway to living well at home. We’re a community-based program for people 55 and older, that promotes health and independence, while helping to prevent or delay nursing home placement. Our Care Resources team partners with you and your family to create a care plan that supports your individual physical, mental, emotional and social needs.

Care is provided through our primary care clinic, day center, home care and trusted community providers – guided by our team of doctors, nurses, therapists and other specialists. Together, we open the doorway to your senior independence.

Care Resources PACE Eligibility

  • 55 & older
  • Meets State of Michigan criteria for nursing home level of care
  • Can live safely at home with our services (at enrollment)
  • All zip codes in Kent County and the following select zip codes in Allegan (49315, 49316, 49323, 49328, 49344, 49348), Barry (49058, 49325, 49333, 48849, 48897), Ionia (48809, 48815, 48846, 48849, 48865, 48881, 49302, 49331) and Ottawa (49345, 49418, 49428, 49435, 49534, 49544) counties.

Participants may be liable for the cost of unauthorized or out-of-network services (except in emergency situations).
PACE participants will receive all needed health care services (other than emergency services) from Care Resources or an entity authorized by Care Resources.
All services must be pre-approved. Not all people in PACE receive all of these services. Services are based on your care plan.
